Required Documentation

11Patient First Wellness Center specializes in Pain Management and Opiate Detox and utilizes a Board Certified Pain Management specialist. Patient First Wellness Center believes that every patient should have choices when it comes to being “pain free” however it is also important for both the physician and patient to abide by the existing laws when participating in the Nevada medical marijuana program. 

Patient First Wellness Center 1.)  will not provide a medical marijuana recommendation to anyone under 21 years of age. 2.) you must have a valid Nevada drivers license. 3.) you must have “written documentation” NRS 453A.170 which is a.) a statement signed by the attending physician of a person diagnosed with a chronic or debilitating medical condition; or b.) copies of relevant medical records of a person diagnosed with a chronic or debilitating medical condition (added to NRS by 2001, 3055). The following documentation is required and/or recommended by Patient First Wellness Center in order to receive a medical marijuana recommendation for a Nevada license.

Pain Management

1.) MRI or CT Scan with positive results (Required)
2.) Prescription Medication Printout from your Pharmacy (Required)
3.) Surgical Report (Recommended but not Required)
4.) Physical Therapy Reports (Recommended but not Required)

Acquired Immune Deficiency Syndrome (HIV/AIDS)

Written reports, lab work and a referral from an infectious disease specialist or internal medicine physician.


Written reports, Lab work and a referral from an Oncologist.


Written report and a referral from an Ophthalmologist.

Seizures and Seizure Disorder

Written report and a referral from a Neurologist
